However, the background is a good example of what grids will do. Which size grid used is impossible to say, as a 10 degree grid could do it if the light was off to camera left (the bottom left side is more intense, indicating direction or a 40 degree will do it if placed just behind the subject. Again, power intensities would vary in those examples, and not in direct correlation to the inverse square law (since a 10 degree grid has more material blocking the light than the 40 degree, the light intensity loss is not dictated soley by distance as it would be with a regular reflector). I would guess not far out of frame, due to the almost circular nature of the spot. The further off axis, the longer the spot gets.
The light coming from camera left and hitting her is probably gridded. I would guess (and its only a guess) maybe a gridded beauty dish.
Another gridded light to camera right and behind her for a kicker.
And a gridded high fill (there are shadows on the camera side of her legs that do not appear on her face).
