It's not always possible, but a habit I got into long ago is to put the softbox face down on the ground/floor once it is assembled, then mount the monolight to the softbox ring, THEN pick the entire thing up and mount it to the lightstand or whatever. Doing this puts gravity to work for you as far as getting a tight, flush fit of the softbox to the Balcar mount.
Doing this, I've never had a softbox come off on me, and I use some fairly large and heavy ones at times. My heaviest is a 59" octa w/standard rods, inner diffuser, outer diffuser, and grid. I mount it with a Bowens QuickRing, which adds even a bit more weight.
I also do not directly remove a large softbox from a mononlight. I lift the whole thing off, put it face down again, and then remove the monolight from the softbox. This avoids a large, heavy mod quickly slipping downward on you and potentially breaking your dome/tube/modeling lamp.
